EVAULATION PROCESS
1. History Taking
A full history will be obtained from past records, questionnaires and
a personal interview with the parents.
2. Examination of the Child
This examination of the child includes not only a general physical
examination, but also a neurologic evaluation and a neurodevelopmental
assessment. The latter evaluates the way a child uses visual,
auditory and tactile input, how he/she coordinates this with stored
information and the quality of verbal, gross and fine motor output. The
child’s behavior, concentration and response to stress and praise are also
evaluated, for the child with physical handicaps, an assessment of range
of motion, body alignment, activities of daily living and equipment
needs is also performed.
3. Laboratory Tests and X-rays
If the history and physical evaluation indicates the need for laboratory tests or x-rays, they will be discussed with the parents and ordered.
All laboratory work will be performed by outside laboratories.
Orders will be provided by
this office, however, this office does not have a laboratory facility.
Abnormal results requiring immediate attention will be reported to you promptly. Other abnormal results will be reviewed with you at your next office appointment.
4. Informing Interview
The results of the evaluation will be discussed with the parents. It is
recommended that children over 16 be included in this process. We
will formulate a plan of action and follow up at this time. The option
of a 2nd informing interview with a younger child is available. There
is a separate charge for this appointment.
5. Written Report
An in-depth report summarizing the total evaluation will be sent to
you and, with your approval, to your primary care physician
6. Outside Resources
Identification of appropriate resources will be reviewed and
discussed during informing interview.
7. Follow-up Visits
These are set up on a periodic basis in order to deal with issues proactively and to monitor your child's progress. There is no set interval for follow up appointments, however, each recommendation is individually determined with the exception of rare cases; children must be seen every four to six months. As children grow and change, treatment programs must be altered accordingly.
